Patient Ragistration

Registration is an important step to becoming an BIHS patient after you confirmed as a Diabetic patient. It helps you and your medical information. Registration includes collecting basic information from you and creating your medical record.

WHERE AND WHEN TO REGISTER

Registering when you schedule your first appointment after you confirmed as a diabetic patient. BIHS Doctor will suggest you with some clinical test and BIHS nurses will save you time and ensure all required paperwork has been completed.

Ragistration Checklist

Doctors will examin you with some Lab tests, You will be also examin by Opthalmology and Dental Specialists during your ragistration. Please have your the information you will need available to complete registration includes:

1. Your full legal name, including maiden name or any other former names, nicknames
2. Your birth date
3. Your street address (or the address of the parents or guardians of minor children.)
4. Your home, work, and cell phone numbers (or phone numbers for the parents or guardians of minor children.)
5. Name, address and telephone number for person who will be responsible for payments (if any)
6. Special requirements (interpreter, stretcher access, etc.)
7. Types of previous injuries (if any)
